While waiting for the next tray of parts to come out of the X-ray, he rested his hand on the side edge of the roller system. The chain/sprocket teeth then caught his right glove, pulling his fingers into the sprocket. Employees were performing a drill pipe inspection using an x-ray/ultrasonic test. A forklift was picking up pipes; its boom pressed down on two joints of pipe, and when the pipes were raised they jumped off the pipe rack, striking and breaking an employee's left tibia and fibula.

An employee from ABM was working in a foundry troubleshooting the door that weighs 300 lbs. on an X-ray unit. The door was stuck and the employee was under the unit troubleshooting the problem when the door released and pinned his forearm under the weight of the door. He sustained nerve damage and a laceration from his left elbow to his forearm that required stitches.

On 7/31/2015, at approximately 7:30 a.m., an employee in the Intensive Care Unit (ICU) was helping a patient turn her body, so that the file film for x-ray preparation could be placed beneath the patient's body. The employee began to walk around the patient's bed when she tripped over IV tubing and fell to the floor. She sustained a right-leg femur bone fracture and bruised her face.

On 3/3/15, an employee slipped on ice in the parking lot and fell. The employee was immediately taken for medical treatment where X-rays showed a broken right arm or humerus. The employee was scheduled to see an Orthopedist on 3/4/15. Following the appointment with the Orthopedist, the employee was admitted to the hospital for surgery on her broken humerus.
