At about 1:00 p.m. on April 28, 2020, an employee was checking a sulfuric acid railcar to ensure that it was empty before it was sent offsite for cleaning and decommissioning. The employee loosened the railcar's dip tube cap with a pipe wrench, then removed it. Residual pressure in the railcar sprayed about 4 pounds of acid into the employee's face, causing chemical burns to the employee's forehead, cheeks and nose.

The employee landed on both feet and sustained a broken left ankle that required surgery.

An employee was performing maintenance on a vertical two-phase separator. The employee was reconfiguring the sight glass and using various wrenches to loosen the existing fittings and tighten the new fittings. The fitting that connected the vessel to the sight glass piping broke behind the closed valve. The swage (fitting that broke) caused the pressurized hydrocarbons to release into the air, which found an ignition source and started a fire. The employee received second and third degree burns

An employee was changing a die inside the pellet mill of a press. The employee was using a torque wrench to tighten the inner bolts, applying pressure in a downward motion, when the wrench either gave way, or reached the desired amount of torque and completed its designed function to break loose. The employee's right thumb was caught between the wrench handle and a piece of adjacent equipment (the aspiration line) resulting in an amputation to the right thumb.

On May 29, 2019, at 1:30 p.m., an electrical maintenance technician was using a wrench to tighten a bolt on a roof exhaust fan assembly. As the employee was tightening the bolt, it broke causing their right hand to strike against the fan assembly resulting in amputation of the ring and index fingertips. The fan's energy source was isolated and locked out at the time.
