She was hospitalized with two broken bones on her left ankle and torn ligaments that required surgery.

He lost his balance and fell to the concrete floor, hitting his head. He experienced dizziness and was hospitalized for a concussion.

When the collar came in contact with the board, it caused the end of the board to rise up, partially degloving the employee's left shin that required a skin graft.

Two employees were assembling doors onto storage cabinets while the cabinets were laying on their backs. After attaching the doors, they stood the cabinet up onto a wooden dolly. The doors came open and the excess weight away from the center of gravity caused the cabinet to fall forward on top of one employee. The employee was hospitalized with abrasions to the left arm and pelvis.

After removing the preventers from the wellhead, they put a wooden board on the ground to set the preventer on and keep it from getting muddy. When one employee unhooked his tagline from a preventer, the preventer shifted and the locking stem on its top struck the top of the employee's left boot. He suffered a laceration to the foot and two broken toes.

Two employees were operating the slitter machine and hydraulic table on Line 1. Employee 1 was operating the controls while Employee 2 was using a wooden stick to pick up the ends of sheet metal coming from the slitter head to set them on the hydraulic table. As the hydraulic table moved towards the slitter head, Employee 2 was caught in between the slitter machine and the hydraulic table. Employee 2 was hospitalized for a contusion and crushed chest wall, liver lacerations and bruised kidneys.
