Two employees were moving a trailer mounted compressor by hand after disconnecting it from a truck. The two employees were pulling the trailer forward, moving closer to the back end of the original tow vehicle. As the momentum behind the trailer increased, employee 1 attempted to control the trailer by placing his hand on the trailer tongue. Employee 1's finger became caught between the towing truck's bumper and the edge of the trailer tongue. The employee' s little finger was amputated.

During the drawing operation, the employee was adding lubrication powder to the die area of the machine when the glove on their right hand was caught and pulled into the rotating spindle. The pinch point where the wire wrapped around the spindle caused a partial amputation of the employee's third digit and a severe laceration to the fourth.

On July 7, 2021, at approximately 5:30 p.m., an employee was parking a front-end loader after moving hot coils from production to shipping in a steel mill. While exiting the front-end loader, the rear wheel on the left side of the loader exploded, throwing the employee approximately 20 feet from the boarding platform. The employee was hospitalized with broken bones in both ankles.

An employee was attempting to move a steel form panel with a crane when the spreader beam became tangled with a rebar cage. As the load began lifting, the tension from the chains making contact with the rebar cage allowed the load to swing toward the employee, trapping him between the steel form and a solid staircasing and resulting in an injury to his left leg.

An employee was screwing in drywall while standing on approximately the middle rung of a 6-foot A-frame ladder. The employee leaned out to the left to reach his work when the ladder tipped over sideways. He grabbed a metal stud with his left hand in an attempt to stop his fall and his foot got caught in the ladder during the fall. He sustained lacerations from the metal stud and a fractured hip caused by contact with the ground. The employee was hospitalized.

After allowing employees to clear approximately 25 feet away from the lift, the forklift operator began to lift a 10-foot section of steel beam. Two employees were changing the cups in a pump. The pump was started up to put the machine in reverse, which would get the cylinder cup out to the end of the material cylinder so the cup could be removed. The swing tube switched, and an employee's right thumb and index finger were caught in it. The employee suffered amputations to the thumb and finger.

The sheaves of the crane were frozen due to ice and snow conditions, which caused a backlash with the auxiliary cable of the crane. The operator attempted to straighten out the backlash on the superstructure near the drum. The frozen sheave freed itself, allowing the cable to run, and the operator's right arm got tangled in the cable. The operator sustained an open fracture of the humerus and a dislocation of the radius.
