The employee picked up the pillow block and while setting it into place, his left middle finger was pinched between a plate hole and the stud of the nacelle bed plate main frame. The employee's left middle fingertip was amputated.

As the employee was stepping from atop one windmill machine head to another adjacent windmill machine head, she slipped and fell 15 feet to the ground below. The employee suffered a compression fracture of the L1 vertebra, a broken pelvis and a broken right ankle.

As the employee was hooking a safety latch to the corner casting of a windmill, the vessel where the employee was working moved a little, creating tension on the hook. The employee's left ring finger was caught in the safety latch, fractured, and the fingertip was amputated without bone loss.
