The employee then attempted to turn left, back down the incline, and the front wheel may have caught on a concrete barrier. The employee fell out of the vehicle, landed on the concrete ground and sustained a broken left femur.

On July 17, 2020, two employees were using a 6-wheeled utility vehicle to assist them in traveling between rows of apple trees to pick up pruned branches from the ground. At approximately 10:00 a.m., the injured employee was picking-up branches and was struck by the utility vehicle. The injured employee was hospitalized with multiple fractures of the sternum, pelvis, and ribs.

An LSC Communications security guard was performing security checks throughout the facility while operating a three-wheeled electric cart. The cart struck a vertical steel I-beam, causing the employee to continue forward and contact the edge of the I-beam and then fall to the concrete floor. The employee was hospitalized with lacerations on the forehead and nose, and fractures to the forehead and right cheek bone.

She fell and hit her head on the concrete floor, suffering a subdural hematoma that required hospitalization

On or about Feb 17, 2015, the injured employee was found by another employee at the bottom of the stairway at a client's facility located at Kasco, LLC (1569 Tower Grove Ave., St Louis, MO). It appears from where the employee was found, that he had attempted to carry five cases of soda up a stairway with a two-wheeled dolly and lost the load. The stairway that was involved in the accident has approximately 16 steps and does not have a landing.
