Employees were pouring a culvert using a trackhoe. The injured employee was dumping a concrete bucket when the hydraulic line on the trackhoe failed causing the stick portion of the boom to release. The bucket struck the injured employee and he fell into a rocky bank and struck his head.

An employee was climbing out of a trench when the bucket of a trackhoe struck the employee's hand, crushing it between the edge of the bucket and the surface of a road. The employee suffered a partial fingertip amputation of the left ring finger.

The injured employee was helping a welder while a nearby trackhoe operator had a 4' pipe joint hooked up to the trackhoe with a strap. The operator leaned forward to adjust his side mirror and as he sat back down, his shirt was caught on the joystick, which caused the machine to move the 4' pipe forward. It struck the 6' main line pipe, which in turn fell off the cones and landed on the injured employee's legs. Both legs were broken and she was hospitalized.

An employee was cleaning mud from a roadway while a trackhoe was being operated nearby. A car came over the hill in the same lane as the employee. When the employee stepped out of the way, he moved in the direction of the turning trackhoe. His overalls were caught by the track, which pulled his left leg underneath. His left leg below the knee had to be surgically amputated.

A trackhoe operator was cleaning out the bed of a Kenworth Tri-axle dump truck. The dump bed was raised and the trackhoe was scraping material out of the bed. The injured employee approached the dump truck while this was occurring and proceeded to stick his head in between the dump bed and the swing gate to see inside. An employee exited his dump truck and approached a trackhoe operator. The trackhoe had a piece of concrete barrier in its grasp about 6 inches off the ground. The concrete barrier rolled onto the employee's right foot and leg, resulting in broken leg/foot.

On March 10, 2015, at approximately 1:40 p.m., an employee was using a tag line to guide pipe being moved by a trackhoe operator.
