On August 10, 2020, an employee was removing tools from a press. He was using his right hand to operate the hoist chain while his left hand was on the eye bolt. When he pulled the hoist chain, it created tension on the eye bolt as it picked up the load. His left index finger was then crushed between the eye bolt and the spinner, resulting in a finger amputation.

He returned to the blower after approximately 10 minutes. The fan/belt/pulley were slowly rotating even though power had been isolated. The employee removed a cover so he could press a tool against the belt to bring the blower to a stop. While reaching for the tool, his hand slipped into the pulley/belt resulting in an amputation to the right

The employee lost consciousness and sustained multiple fractures to his skull, face, shoulder and ribs. The employee was wearing personal protective equipment at the time.

On October 5, 2017, at approximately 9:00 a.m., three employees were working together to remove vinyl siding from the side walls of a residential balcony for demolition. They were working from extension ladders with hand tools. The injured employee entered the balcony from the extension ladder to look for a potential anchor location for his personal fall arrest system. When he entered the balcony, it collapsed beneath him. He landed on the ground and broke his leg.

On 9/9/2016, at approximately 1:30 PM, an employee and coworker were trying to uncrate electrical switchgear. They were using tools to pry the switchgear's cabinet off the wooden pallet when the switchgear shifted over onto the employee's right foot and knocked him backwards, leaving him hanging over the mezzanine by his caught foot.
