An employee was aligning a conveyor with a press used for laminating plywood. When the employee kicked the conveyor, his right foot was pulled into a chain and sprocket. The employee attempted to free his foot with his right hand which was also pulled into the chain and sprocket. The employee sustained the amputation of two toes on their right foot. The chain and sprocket were not guarded at the time of the incident.

While hooking up a crane, the crane rigger signaled the crane operator to slack off. The operator slacked off and one chain was removed. When the rigger went to remove the second chain, the counter balance fell to the ground. The counter balance hit the rigger's left foot and severed their boot, resulting in injuries to the big toe and the toes next to it.

An employee was moving a small container to a staging area with a forklift. The employee lowered the container to the ground, but one of the forklift's forks jammed in the wheels of the container. The employee exited the lift and got the container to release. The pressure on the forks released and the left blade fell, landing onto the employee's big toe less than 2 feet below and cutting it. The employee was hospitalized. The toenail was removed.
