The forklift spun and struck two co-workers, knocking them approximately 4 feet off the dock. The forklift fell off the dock and landed on the injured employee's right thigh, resulting in a fractured leg.

On June 30, 2021, at approximately 4:43 a.m., the injured employee was attempting to remove a 3/4" carbon steel pipe that was stuck in the furnace. A co-worker assisted the employee by operating a forklift with a boom attached to hoist the pipe from the furnace. The injured employee was holding one end of the pipe when the pipe fell from the boom and struck the employee's left thigh. The employee suffered a hematoma on the left thigh and was hospitalized.

He suffered a broken jaw and bruising to his face, shoulders, and thighs.

He descended a fixed ladder with assistance and was hospitalized with electrical burns to the left arm, buttock, and thigh.

The employee sustained second degree chemical burns from the lime to the neck, abdomen, both upper both arms, both thighs, and both ankles, as well as third degree burns on one ankle.
