When the piece touched down on the deck it slid and rolled as the employee was moving to another area. An employee was standing in front of a forklift carrying several 2,000-pound "logs" of aluminum. The logs were banded together; when the employee cut the banding so they could be unloaded, one of them rolled onto the employee's foot. The employee's steel safety toe collapsed, and the employee's toes and metatarsals were crushed. Part of the foot was medically amputated.

An employee was guiding a chiller head as it was being hoisted by a chain hoist. A strap that was used to hang the chain hoist snapped, causing the chain hoist and the chiller head to fall. As it was falling the chiller head hit a section of steam piping and veered off towards the employee, landing on the bridge of his right foot. Four metatarsals were each fractured in two places, requiring hospitalization.

The employee touched a bail that had a sling wrapped around it and the bail fell to a track. The bail brushed the employee’s shin and then landed on his right foot. As he pulled his foot out, he fell backward about 5 to 7 feet away from the bail. He was hospitalized with a bruised metatarsal and tarsal on his right foot and

A caster operator was removing a safety blank while wearing heavy welding gloves to insert a smaller nozzle in a casting tundish holding molten steel. The blank fell onto the top of the shroud, causing the molten steel to splatter onto his flame-resistant clothing and metatarsal safety boots. He suffered second- and third-degree burns to his lower legs and feet.

When the employee lifted the block approximately 3" from the pallet it was sitting on, the block pivoted, fell from the strap, and struck the employee on his right foot. His 1st, 2nd and 3rd metatarsal bones were fractured, requiring hospitalization and surgery.

When he cut the fourth and final strap, the top bar sank down between the two bottom bars and one of the bars rolled onto his foot, fracturing his first and second metatarsals. He was hospitalized.

He suffered metatarsal fractures and tarsometatarsal dislocations.

He was hospitalized overnight for surgery.

On March 28, 2016, at approximately 11:20 p.m., an employee exited his excavator and peered into the bin where he was placing additives. Wearing metatarsal gloves, the employee placed his left hand on the side of the bin. A piece of metal, used as a trough, was not secure and slid against his hand, amputating his left pinky finger down to the knuckle.
