While he was trying to get the cable back on track, tension developed in the cable, severing the ring, middle, and index fingers of his left hand and the index and middle fingers of his right hand.

The employee was hospitalized overnight for a fractured vertebra. The coworker was not hospitalized.

An employee was working on a bakers scaffold about 4' high, patching the fireproofing on structural steel. The employee stepped off the scaffold and fell backwards. The employee possibly experienced head and back injury and was taken to the hospital. At the hospital, the employee was treated and admitted for a finger infection. The employee used his hand to brace the fall and aggravated a previous injury.

On or about March 20, 2015, at 3:30 p.m., an employee was placing/helping to place a structural tube on a concrete pier foundation at the jobsite. While the tube was being set, the bottom of the column slipped off of the pier and hit the employee on the ankle. The structural tube pier weighed approximately 250 pounds and was approximately 14 to 16 feet long. This structural tube would be used as a vertical support tube.

On February 20, 2015, an employee conducting preventative maintenance work on a heater was injured when the scissor lift he was standing on was struck by the structural steel beam for a gantry crane. He suffered multiple fractures on both sides of his body, as well as a dislocated left hip, when he fell approximately 15 feet to the ground.
