The material struck the employee's chest, cracking ribs and puncturing a lung. The employee was hospitalized.

When he was repositioning his hand, he grabbed the side of the handle and hit the crimp mechanism. The crimp mechanism clamped down on his left-hand index finger, amputating the finger from the nail to the first joint.
