An employee was adding a strand of bad wire to strands of existing wires that were being pulled onto a de-spooler. As the employee added the bad wire to the existing wires, the employee pushed down on the strands of wires which created a loop. The loop grabbed the employee's gloved left index finger and pulled the employee's left hand into the guides, resulting in a left index fingertip amputation.

The kelly cable drive was not retracting properly on the machine's drum, so the employee tried to disconnect the cable to re-spool the drum. He backed out the threaded pin from the kelly drive clevis that was holding the cable drive end. As he tried to push the pin through, his right middle finger was pinched in the clevis and cable. The finger was amputated at the first joint.

The employee was going to use scissors to cut the tubing off the winding reel. The employee suffered the amputation of the left middle fingertip.

As the outrigger was retracting, the employee's left hand and arm were caught between the outrigger base and the crane outrigger causing a crushing injury to the employee's left hand/wrist that resulted in surgical amputation.

The liner was slipping. The employee reached in to secure it from slipping and his hand was caught between the liner and the rotating spool (bobbin). This resulted in a fracture of both bones in the lower left arm which required surgery.

The forklift raised the pipe and when the employee moved his hand back and he suffered an amputation.
