Two employees were unclogging the chipper after a rainstorm. One employee opened an access panel to the drum of the chipper to unclog it while the second employee went to the other side of the chipper. The first employee then shut the access panel. The motor was off, but the drum was still spinning. A piece of clogged brush kicked the panel open and struck the second employee in the forehead, resulting in multiple skull fractures.

The shear blade was not guarded at the time.

An employee was about to start winding the core of a hose on a reel, which requires attaching the end of the core to an air inlet on the side of the reel. The reel becomes automated once the hose core is attached and begins to spin. When she went to attach the core to the air inlet, the reel began to automatically spin. Her hand was caught between the spinning reel and a clamp that holds the pin in, resulting in a fracture to the second metacarpal bone of the left hand.

The employee was in line with the V-roller line which brings pipe to the rotation station. The V-roller line engaged, sending a pipe in a south to north track. The employee was caught between the moving pipe and the pipe that was on the rotating wheels. The employee was hospitalized for an abrasion.

The employee sustained multiple fractures to their arm.

An employee was cleaning and clearing regrind from the area around a large mechanical grinder. The grinder was powered down and locked out with the machine guarding and covering removed. The employee spun the shaft of the grinder by hand to check and see if the shaft/rotor was clear. This caused the shaft to rotate. He then tried to stop the shaft from spinning and his right hand contacted a blade attached to the shaft, causing multiple lacerations to his fingers.

A crane operator was lifting a vertical pipe inside a steel tower structure so the pipe could be cut to size when the pipe started spinning. An employee went to check on the pipe when it came loose and fell approximately 6 feet, crushing the employee against the tower structure. The employee suffered injuries that resulted in a medical amputation of the left hand and left leg.

On February 17, 2018, an employee was injured while using a drill press. The drill press was set up with a paddle in lieu of a drill bit. While adjusting the speed of the press with his left hand, his right hand held a tachometer. With the paddle spinning, the shaft bent 90 degrees and contacted the employee's right ring finger, resulting in amputation.

An employee and a coworker were removing a 2,500-pound block of concrete using a front-end loader that was hooked to a crane. The coworker was driving the loader while the employee guided the concrete when a corner of the block hit another block and started spinning around. The employee was attempting to stop it from spinning around when the chain came undone, allowing the concrete to fall on and fracture his right leg.
