The motor was off, but the drum was still spinning. A piece of clogged brush kicked the panel open and struck the second employee in the forehead, resulting in multiple skull fractures.

A crane operator was lifting a vertical pipe inside a steel tower structure so the pipe could be cut to size when the pipe started spinning. An employee went to check on the pipe when it came loose and fell approximately 6 feet, crushing the employee against the tower structure. The employee suffered injuries that resulted in a medical amputation of the left hand and left leg.

An employee and a coworker were removing a 2,500-pound block of concrete using a front-end loader that was hooked to a crane. The coworker was driving the loader while the employee guided the concrete when a corner of the block hit another block and started spinning around. The employee was attempting to stop it from spinning around when the chain came undone, allowing the concrete to fall on and fracture his right leg.
