A forklift operator had just placed a 3-high stack of bundled lumber on the ground. He then backed the forks out from underneath the bundles and exited the forklift. He used a pair of snips to cut the nylon banding off the top bundle. The top bundle moved and the bottom board fell to the ground. As the employee moved away from the stacked bundles, he tripped over the forks and fell to the concrete ground. The employee was hospitalized with fractures to the right ilium and femur.

An employee was travelling in an ATV to the worksite. The ATV passed through tall grasses, fireweed, and cow parsnip. The employee had an allergic reaction to the local vegetation which affected the eyes, skin, and airway and required hospitalization.

An employee was working from a ladder to remove ductwork from the ceiling. He snipped a hanger on one end at the joint and the duct joint gave way, causing the ductwork to drop at one end and swing back, striking the ladder at the base. This knocked the ladder out from under him, and he fell to the floor, landing on his hands and knees at an angle. He broke his arm, fractured his elbow, and hurt his knee.

An employee was setting up a rolling mill. Once he properly sized the wire, he went to cut the nonconforming wire off. At this point, the machine was stopped and he released the tension. He held the loose material in his left hand and went to reach for his snips. As he was holding the material, tension was engaged, he felt a pinch of his left ring finger and instinctively pulled his hand away. As he did so he sustained a partial amputation to his left ring finger.

An employee snipped a metal band that restrains a steel coil from unwinding to remove it from the coil. The employee experienced some resistance and pulled the metal band to free it. The metal band held firm and sliced through the employee's protective glove and into his hand, lacerating it.

An employee was measuring plywood with a coworker when the coworker stepped on the plywood. The injured employee's finger got caught between two sheets of plywood and the left ring fingertip was snipped off. The employee required surgical amputation down to the first joint.
