On April 3, 2019, at approximately 2:00 AM, an employee was MIG (metal inert gas) welding on a steel bridge cord. A piece of hot silica (slag) fell into the employee's left boot causing a severe burn to the bottom of the foot.

On 07/23/18, an employee was working with a grinder and low carbon cast steel shot that contained iron, manganese, carbon, and silica. He experienced respiratory issues and leg and foot swelling. He was hospitalized for congestive heart failure.
