On October 27, 2021, two employees were assembling a shelving unit with a co-worker. A boom attachment on the top shelf of the racking fell down as the two employees were pulling on the crossbar to lower one of the shelves in the middle of the rack. The boom fell and hit the crossbar, jamming it in an employee's left collar bone and chest. The cross bar then fell and landed on the other employee's feet, resulting in a fracture and hospitalization.

He suffered nerve damage to his thumb and thermal burns to his forearm muscle, and was hospitalized.

When he used the crowbar, the load began to shift on the shelf and he jumped from the ladder to avoid it, landing on his left leg on the concrete shop floor. He fractured his left knee and tibia and was hospitalized.

An employee was cleaning up for the day and was setting a metal shelving unit onto the bucket of a backhoe machine. The employee lost control of the shelf and it dropped, pinching his index finger between the bucket and shelf. The finger was amputated at the first joint.

An employee was squatting down adjusting inventories on lower shelves while a coworker was stocking yeti ice chests approximately 16 feet above the injured employee. The yeti ice chests were in cardboard boxes and weighed about 45lbs. The coworker lost their grip on one of the yeti boxes and it fell onto the injured employee knocking her into the shelving and then onto the floor. The injured employee passed out, and sustained a laceration on her left temple. She also suffered a broken right ank

An employee was working on the roof taking photographs and measurements when he stepped through a skylight and fell approximately 20 feet, striking metal shelving before landing on the concrete flooring. He suffered fractured ribs (all), a fractured sternum, a fractured back in three locations, and a fractured skull.

Employee 1 was in front of an electric pallet jack picking groceries from shelving. Employee 2 was in front of Employee 1 with a pallet jack doing the same. Employee 1 thought Employee 2 was going to move forward, but Employee 2 didn't. Employee 1 kept moving forward with his pallet jack and got wedged between the two pallet jacks breaking his ankle.

Two employees were removing sheets of plywood from old shelving when a piece about 3' by 6' slipped. The injured employee's right pinky fingertip was caught against the plywood and the skin tore off (no bone damage) within the employee's glove. The employee was hospitalized.
