An employee was replacing the shell body of a ride vehicle. He was maneuvering in-between the vehicle and the body to hook a sling to the body prior to lifting it onto the vehicle. He had one hand on the shell body and was stepping over the corner of the vehicle chassis. As he reached for the farthest hook-up point, the shell body shifted and he fell back. Two employees were fabricating a 50,000-gallon steel storage tank and were putting the last head on the tank. One employee was on top of the tank, jacking the head seam tight, when the lug where the jack's hook was attached tore free from the weld on the shell.
