The sheaves of the crane were frozen due to ice and snow conditions, which caused a backlash with the auxiliary cable of the crane. The operator attempted to straighten out the backlash on the superstructure near the drum. The frozen sheave freed itself, allowing the cable to run, and the operator's right arm got tangled in the cable. The operator sustained an open fracture of the humerus and a dislocation of the radius. | |||||

On January 28, 2021, a crane block and hook had been lowered to the ground for removal from the crane. As the crane load hook came in contact with the ground, an employee tried to rotate the load hook so the block/sheaves would come to rest in a vertical position on the ground. The hook became unstuck and his right foot was wedged between it and the load block. His little toe was broken. | |||||

Two employees were performing maintenance on a lift by a tower and were being transported in a basket up the lift line. When the basket arm contacted the sheaves of the tower, the basket jettisoned from the cable. The two employees fell with the basket 25 feet to the ground.
