The employee noticed a crooked tray on a buggy entering the separator and placed his right foot on the chain conveyor to reach into the machine and straighten the tray. The next buggy moved forward and caught the employee's leg against the conveyor fracturing his tibia and fibula near the ankle.

An employee was manually rotating the rubber ironing roll of a separator to back-thread the tail through the machine in order to make a splice. On August 3, 2017, at approximately 9:00 a.m., an employee was tending a well while standing between two gas production units approximately 15 feet from a sand separator. At that time, a threaded pipe that was going into the top of the sand separator blew apart. The pipe separated at a 90 degree elbow and released approximately 2,300 lbs. of pressure. The gas from the line projected down towards the ground and the employee's skin was struck by ground debris. He was hospitalized.
