At 12:04 p.m. on June 21, 2021, an employee was dumping 50-pound bags of ground caraway into a hopper through a screen. The product bridged in the hopper, so the employee slid the screen out of the way and began pushing the product down. The screw feed auger at the bottom of the hopper nipped his right index finger, causing an amputation.

They had just made the connection. An approximately 60-foot fiberglass rod unscrewed from the holding, slipped out of the transfer, fell out of the barrack, and hit the rig floor. It then bounced off the rig floor onto the ground and struck the injured employee on the left side of the head, knocking him unconscious.

An employee was trying to free a screw that was clogged with rice hulls. The employee shut off the power to the screw and went to pull on the belt that was connected to the pulley that ran the screw. While the employee was pulling on the belt, the screw became free. The stored energy from the bound screw caused the belt and pulley to rotate while the employee was holding the belt. The employee's left index finger entered the pinch point of the belt and pulley, amputating the left fingertip above

An employee was standing about two-thirds of the way up on a 12-foot portable ladder. He was using the claw of a hammer to partially loosen a screw that was embedded on a wooden board attached to the wall. When the screw came free, the employee lost his balance and fell off the ladder landing on his feet on the concrete floor. He sustained a left heel fracture and a right tibia/fibula fracture.
