He was hospitalized.

On September 24, 2020, at about 7:20 AM, an ATV unit agent was assisting in the apprehension of three suspects. While working the group via the ATV, one suspect ran out of a cornfield in front of the ATV. The agent applied the brakes to avoid striking the suspect, flipping the ATV in the process. The ATV then rolled over the agent. The agent was hospitalized for a fractured scapula.

The injured employee was part of a crew unloading a semi-trailer of a 6-foot stack of insulated pipe. After cutting one of the four steel bands, the employee stepped onto a nearby 4x4x4 crate in front of the trailer. The employee lost balance, fell and landed in between the tractor and the trailer. The employee injured their collar bone and scapula and sustained a cut on the head and a puncture wound on the back.

An employee was changing a bottom hole assembly and laying down a stabilizer and pony sub. Nearby rig hands could not get a pipe to a trowel. He saw they were in trouble with the pipe and attempted to help get the pipe to the trowel, but the driller did not land the pipe in the trowel. The pipe landed on the edge and rolled out, pinching him against a frame and stabilizer. He fractured his scapula and the spinous processes on his vertebrae.

An employee was driving a side-boom operator across a temporary bridge. The bridge collapsed and the machine rolled slightly and came to rest at about a 45 degree angle. As the employee disconnected his seat belt, he fell from the machine and landed in the creek bed on his back. The fall caused hairline fractures to his scapula and a rib.

The coworker lost control of the duct work and it swung down and knocked over the ladder, causing the employee to fall to the ground. When he twisted the radiator cap, the cap came off and he was sprayed with hot coolant resulting in burns to his right arm, shoulder, chest and scapula. Immediately afterward he ran towards the rig manager's trailer and while doing so tripped and fell resulting in a broken right clavicle and cracked scapula.

An employee and a coworker attempted to re-activate a dock plate after servicing the hydraulic pump. The employees removed their lockout devices and found that the dock plate would not lower. The injured employee went underneath the dock plate to attempt to adjust it to lower properly. As he was in the process of doing this task, the dock plate fell onto his body, puncturing his lung, breaking ribs, and breaking his left scapula.

The employee and a second employee were confirming measurements adjacent to an open stairwell that did not have the stairs installed yet. The employee suffered a broken left scapula, fractured ribs on his left side and fractured vertebrae in his lower back.

He was hospitalized.

An employee was climbing a fixed ladder on a tanker trailer when the ladder rungs broke, causing the employee to fall backwards. The employee sustained broken ribs and a fractured scapula.

While an employee was moving lumber in row one using a diesel-powered forklift, the top two packs of lumber in row two fell onto the forklift, causing the forklift cage to fail. The cage then struck the employee, fracturing his scapula and dislocating his collarbone. He was hospitalized.

Employees were moving equipment into the trailer and removed the stairs to the trailer. Upon exiting the trailer, an employee forgot the stairs were removed and fell about 3 feet to the ground. Employee has 2 broken ribs, broken scapula, and a dislocated shoulder.
