On September 14, 2021, an employee was at the ground level of a tank performing "ground watch" for an articulating lift. Two employees were on the dome of a tank sandblasting. A sandblasting hose fell off the edge of the tank 100 feet above and struck the employee, who suffered a back laceration, a broken collarbone, and broken ribs. The employee was hospitalized.

An employee was sandblasting the external hull of a battleship. The blasting hose lost pressure because the blasting hose was under the tire of an aerial lift. The employee lowered the lift and tried to free the hose when it ruptured, causing a laceration to the employee's right arm.

On May 14, 2021, at approximately 8:30 a.m., an employee was performing sandblasting operations on the inside rear of a trailer. The trailer was secured to a fixture that allows the trailer to be rotated in order to access different parts of the trailer frame. The counterweight had not been added to the frame or fixture and when the employee began to rotate the trailer, the trailer swung down and collided with their left ankle.
