An employee was working in the mechanical room doing an inspection on the air handler (checking belts, bearings, pulleys, etc. for vibration). He put his left hand on the inboard bearing and was rolling the belts to rotate the shaft while feeling the bearing for abnormal vibration or noises. The belt grabbed his glove and pulled his right hand into the groove of the pulley and belt. He sustained lacerations at two fingertips on the right hand without bone loss.

His left hand slipped and entered the motor where the impeller blades struck and amputated the fingertips of his index, middle, and ring fingers.
