On November 1, 2021, an employee was attempting clear a jam in a riveter when his right middle finger became caught in the machine. The employee's right middle fingertip was amputated and his right ring finger was crushed.

A temporary employee was using a de-riveter machine to remove rivets from a brake shoe. The employee placed a sheet of material down on the table of the de-riveter and then placed the brake shoe on top of it. The employee held the brake shoe in place on the table and engaged the de-riveter. During the machine's cycle, the brake shoe shifted on the surface and crushed the employee's right ring fingertip between the shoe and the de-riveter table, resulting in amputation of the fingertip.
