An employee and two co-workers were removing a feed hopper due to a piece of hard resin that had accumulated between the hopper and the mixer. When the hopper was dislodged from the mixer, it pivoted into a new position and contacted the employee's right pinky finger, crushing it between the hopper and the mezzanine floor and resulting in crush injuries and lacerations that required hospitalization.

An employee was helping to troubleshoot a resin conveying butterfly valve for proper operation. To perform the operation check, the employee had to support the weight of the valve to minimize the strain on the valve's wiring harness. His right thumb inadvertently entered the butterfly valve's area of actuation, and the butterfly valve smashed his right thumb diagonally across the nail, resulting in an amputation.

After cleaning out melamine resin lines that were blocked, an employee wanted to verify that a control valve was operating properly. While standing on a 4-foot ladder, the employee reached over to the far side of the valve and manually actuated the solenoid, causing the valve to open. The employee's right middle finger was in the valve to verify it was open when the employee lost their footing and released the solenoid. The valve then closed and amputated the right middle fingertip.
