While pushing the die set in, the employee’s left hand was on the bottom of the die set, with the tips of his fingers in a recessed hole underneath the die set. As the die set slid into place, the employee's fingertips were contacted by the straight edge of the lower base of the cavity, resulting in the amputation of the index, middle, and ring fingertips.
