At about 3:00 p.m. on October 28, 2021, an employee was on a work platform, which had been raised by a forklift 8-10 feet off the floor so he could access the control box for an overhead door that had stopped working. A door on the platform swung open and the employee fell backward to the floor, landing on the back left side of his body. He suffered six broken ribs on the left side and was hospitalized.

On October 26, 2021, at approximately 10:25 a.m. an employee was performing roofing work on a commercial building and was walking backward moving a propane tank with a handcart. While moving the propane tank, the employee tripped over the raised lip of an unprotected skylight and fell through it to the concrete floor 20 feet below. The employee sustained a fractured cervical vertebra and multiple skull fractures.

An employee was standing under the slip sheet attachment of a forklift, which was raised in the air as the employee installed a lift chain. The push-pole attachment fell on the employee and resulted in severe injuries to the shoulder, back, legs, and hip. The employee was hospitalized.

An employee set up an aerial lift with two outriggers positioned on a road and the other two on an embankment. The employee raised the aerial lift approximately 45-50 feet high to prepare to trim a tree when the embankment gave way and caused one of the lift's legs to slide. The lift fell over and the employee landed on the ground outside of the basket, resulting in compression back fractures at the L1 and L4 vertebrae and damage to the L1, L3, and L4 vertebrae that required hospitalization. While working in the Rigger Shop, an employee was using channel lock pliers to disconnect two pieces of pipe that were bolted together. The pipe was raised by a crane. When the top bolt was removed, a portion of the pipe was pulled upward, and the other portion fell to floor. On 9/5/2020, at approximately 10:40 a.m., a Lowe's employee was making a delivery at a customer's home. The employee had positioned a washer on the lift gate of the delivery truck and was attempting to step around the washer when he fell off the lift gate, which was raised several feet off the ground. As the employee fell, he grabbed the washer and pulled it down on top of him. The washer landed on his abdomen. He was hospitalized with fractured ribs, a fractured C7 vertebra, and a head injury.

He sustained amputations to the middle, ring, and pinky fingers and was hospitalized.

He then climbed down the first section of a fixed ladder from the roof and was standing on a metal platform before climbing down the second section of the ladder. He fell through the opening in the platform where the second section of the ladder starts, and landed on the ladder's base, which is a raised metal platform. He sustained fractures to the left arm and right ankle.

On March 1, 2020, at approximately 5:40 a.m., the injured employee was catering a flight with employees 2 and 3. While the catering box on the lift truck was elevated, employees 2 and 3 were closing the aircraft's door and heard a noise. They observed the injured employee on the ground below. It is believed that the injured employee was inside the catering box while it was raised to service the aircraft and fell out after the box's door was opened. The injured employee sustained a skull fracture

An employee was loading a roll of paper (500 to 800 lbs.) from a railcar onto a powered industrial vehicle (PIV) when the roll fell on top of the employee's PIV protective cage. The employee raised his hand to protect himself and his fingers went outside of the cage and were crushed between the bar of the cage and the roll of paper. As a result, he sustained a partial amputation of his left index finger and debridement of his left middle finger.

While the employee was standing at the rear of the truck, the asphalt broke loose and fell onto the employee, pinning the employee between the pavement and the tailgate of the dump truck. The employee suffered burns, lacerations, broken bones, and the amputation of toes.

Tri-axle dump trucks were importing stone to the worksite. After one of the trucks emptied its load the injured employee noticed stone on the tail of the body where the tailgate would close. The employee notified the driver who, again, raised and lowered the bed of the tri-axle to dislodge the debris. When the bed lowered the employee's left hand was caught in a pinch point between the truck body and the tailgate resulting in amputation of the middle and ring fingertips.
