An employee was packaging PVC pipe on an extruder when the PVC pipe became misaligned on the transfer arms prior to the chamfering process. On April 22, 2016, at approximately 7:05am, an employee was hospitalized after being injured while moving a plastic water line on I-beams. The employee and a coworker were rolling a 14 inch diameter PVC pipe weighing approximately 300 lbs. and 15 feet in length across I-beam stands. During the roll, the pipe slipped off the I-beam and the employee's thumb was pinched between the I-beam and the pipe resulting in cuts and crushing injuries to the left thumb.
