While he was attempting to remove the bolted-on steel plate, the plate (which was under pressure) blew out and struck the employee on the right hand and dorsal forearm. He sustained three broken fingers on his right hand, a torn-off fingernail, and lacerations to his right thumb and left dorsal forearm.

On April 22, 2016, at approximately 7:05am, an employee was hospitalized after being injured while moving a plastic water line on I-beams. The employee and a coworker were rolling a 14 inch diameter PVC pipe weighing approximately 300 lbs. and 15 feet in length across I-beam stands. During the roll, the pipe slipped off the I-beam and the employee's thumb was pinched between the I-beam and the pipe resulting in cuts and crushing injuries to the left thumb.

The employee then contacted a live, energized electrical conduit and was electric shocked, suffering nerve damage and leg pain.

The finger tip was recovered and delivered to Parkland Hospital, but didn't have enough bone to be reattached.
