Two employees were breaking a seal between the pulverizer and the pyrite seal box using a prybar. Once the seal was broken, a five-inch gap was left between the two. The injured employee's left hand was in the gap between the pulverizer and the seal box. As they slid the box, it dropped three inches off the shims and onto the injured employee's left hand resulting in an open fracture and amputation of the pinky fingertip.
