An employee was adding a strand of bad wire to strands of existing wires that were being pulled onto a de-spooler. As the employee added the bad wire to the existing wires, the employee pushed down on the strands of wires which created a loop. The loop grabbed the employee's gloved left index finger and pulled the employee's left hand into the guides, resulting in a left index fingertip amputation.

The kelly cable drive was not retracting properly on the machine's drum, so the employee tried to disconnect the cable to re-spool the drum. He backed out the threaded pin from the kelly drive clevis that was holding the cable drive end. As he tried to push the pin through, his right middle finger was pinched in the clevis and cable. The finger was amputated at the first joint.

An employee was preparing a spool to take up cable from an extrusion process. Tubing got caught on the collet and spindle of the winder. The employee was going to use scissors to cut the tubing off the winding reel. Before being able to cut, the tubing tightened on the rotating spindle shaft, amputating the employee's left index and middle fingers at the first joint.

An employee was preparing to repaint the indoor pool. The pool had been drained and a scaffold was erected inside of the pool. The employee leaned over the railing to drop a broom down to another employee working below when the midrail on the scaffold failed causing the employee to fall 12 feet before striking cross bracing and then fall another three feet to the bottom of the pool. The employee was hospitalized for a broken wrist and internal abdominal injuries.

The employee suffered the amputation of the left middle fingertip.
