On November 6, 2021, an employee was spray painting a petroleum pipeline for rust prevention. The pipeline is supported by wood log cribbing and subject to move when the temperature rises. The pipeline fell and pinned the employee down in the excavation. The employee sustained several broken ribs, breathing obstruction, contusion, and loss of consciousness. He was hospitalized.

On 10/15/2015, at about 10:00 p.m., an employee was taking a sample from a petroleum unit of the refinery. During the sampling process, the glass bottle ruptured and broke. The glass debris injured the employee's face and left eye.
