An employee had been manually loading a wooden shoe holding device with a die-cut urethane foam piece. As the employee moved the shoe into the cutting machine along a fixed pathway, one of the saw blades came in contact with the shoe, causing the 4" blade to break into many small pieces. One or more of these pieces struck the employee’s fingers. The right-hand pinky and ring fingers sustained cuts that required stitches. The ring finger also required surgery for a cut ligament and blood vessel.

The injured employee was supervising another employee who was operating a track skid steer loader during a backfilling excavation. The ram hit the edge of the pin and flung it out violently. The 1-pound metal pin hit the employee's head, causing facial lacerations and a skull fracture.

When he moved the level to dump the hopper, the weight caused the hopper edge to drop and crush his left pinky fingertip, which had to be surgically amputated.

A mulching block-off plate was not in place on the mower's discharge opening and pulley guards were not installed at the time of the incident.

On an access road, a front-end loader operator lost control of his vehicle and slid into the path of the injured employee, who was driving a garbage truck. The front-end loader crushed the garbage truck cab, trapping the injured employee and crushing the employee's left leg.

An employee was driving a golf cart. When he attempted to turn right, the golf cart left the paved path and his right knee hit a trash can. He attempted to hit the brake, but instead stepped on the accelerator. The golf cart flipped on top of him and injured his back, requiring surgery.

An employee was raking a bunker (sand trap) with a mechanical rake (unit is ridden by the operator). There are three individual bunkers at the #11 Green, which are above a retaining wall approximately 90 feet high. The employee was riding the rake on the slope towards the retaining wall and felt the machine slipping. He disembarked from the rake on to the cart path below the wall and fell fracturing ribs on his left side.

On 9/7/15, an employee was cleaning out top flash and trimmings that had fallen on the floor beneath an auto-trimmer. His finger entered the stroke path of the lift cylinder, and his right middle fingertip was amputated.

An employee was attending training and started feeling queasy/lightheaded during the module on blood-borne pathogens. He left the room and sat in the northeast stairway. A short time later, he fell down the stairs and was found unconscious on the landing below by a coworker. His coworker woke him up and asked if he was okay. The injured employee stated that he was okay, just tired. He was later hospitalized for a skull fracture and concussion.

An employee was using a forklift to transport cardboard from the rear of the building to a storage area. He did not see a structural pole in his path of travel due to a blind spot created by the forklift mast. He struck the pole and was ejected from the forklift. He was not wearing his seatbelt. His head struck the mast of the forklift as he was ejected. He was hospitalized.

On May 1, 2015 at approximately 11:00 AM two employees were riding in an off-road ATV along the Lechate levee pond when the driver hit a rut and lost control of the vehicle. This caused the vehicle to slide and flip, ejecting the driver into the path of the roll over. The driver sustained injuries to his jaw, collarbone, ribs and internal bruising to his liver. The passenger was not injured.

An employee was oiling the conveyor chains at the end of the main saw roll-case (conveyor) near the lumber backstop. When lumber from the main saw reaches the end of the roll-case conveyor near the backstop, the lumber trips a lever arm limit switch, activating a lumber sweep that pushes the lumber onto a perpendicular conveyor. On 2-2-2015 at about 8:30 a.m., the injured employee was driving a baggage tractor, towing four empty dollies. A forklift was being driven forward with cargo; the operator could not see the travel path and collided with the tractor. The injured employee experienced internal injuries to the stomach from impact with the tractor wheel. The employee was taken to the hospital and is in stable condition.

The employee had finished making a drop off at the Waste Management Landfill, when he observed his back gate was open. He got out of the truck, and went to the back of the vehicle to close the gate mechanism. The truck began to roll backwards, and the employee ran to get back into the vehicle. The employee fell, landing in the path of the vehicle. The vehicle ran over the employee, causing multiple fractures on his right side and a cut to his calf.
