Three employees were emptying a full container of trash into an outside dumpster. As they were lifting the container over the edge of the dumpster, it came down and caught one employee's right hand against the dumpster's edge. The employee's middle and ring fingertips were amputated, including the nails.

An employee was attempting to put a muzzle on a cat in order to trim its nails when the cat bit the worker, resulting in a puncture wound to the right index finger that became infected and required hospitalization.

An employee was positioning a steel beam to be welded. As the beam was being lifted to turn over, it got caught on rollers. It came loose while the employee was trying to dislodge it, and his left hand was caught between the beam and a roller. He suffered amputations to the first and middle fingers just below the nails.

An employee was bending metal in a 50-ton press brake. His finger was caught in a pinch point resulting in amputation of two fingertips and the loss of two fingernails.

On August 17, 2020, an employee was using a pry bar to remove nails from metal on the edge of a roof. He fell off the roof to the concrete ground below, suffering two broken wrists, broken ribs, a neck injury, and a collapsed lung.

An employee was going to hammer protruding nails into a wall in preparation to take down shelving. The nails were more than 10 feet above the ground. The employee was standing on a wooden pallet which was raised on the forks of a forklift when the pallet failed. The employee lost balance and fell more than 10 feet to the ground. The employee sustained a broken left ankle.

A roofer was carrying a box of nails when he fell off of the roof of a one-story house and landed on the ground. He sustained a broken left arm and left pelvis and was hospitalized. A safety lanyard was not connected at the time of the incident.

An employee was using a 2x4 to help guide an injection core block onto some shelving when the board slipped, resulting in the block smashing his fingers. His left ring and pinky fingers were severed at the tips with about half of his fingernails removed.

An employee was pushing a large lawn mower to hitch it to a tractor. His hand was on the mower's hitch when the mower moved, fracturing and partially amputating his left ring finger and cutting his middle finger. His ring and middle fingernails had to be surgically removed.

On September 22, 2017, at approximately 2:15 PM, an employee was helping a coworker drive nails into a wood door using a nail gun when the door moved. The employee was then shot by the nail gun. The nail pierced the employee's left forearm to the bone.

An employee was removing the nails securing two scaffolding planks together. A coworker had removed the support beam beneath the scaffold platform. When the employee removed the nail holding the two overlapping scaffold planks together, the planks collapsed, and the employee fell 8 feet to the concrete floor, face first. The employee suffered a broken right upper arm, broken cheek bones, and a broken nose.
