The motor was off, but the drum was still spinning. A piece of clogged brush kicked the panel open and struck the second employee in the forehead, resulting in multiple skull fractures.

One employee was hit by the flames, and sustained burns on the neck, left hand, and both legs.

The employee lowered the tank, reset it on the lift, and began to raise it again when the motor fell approximately 10 feet from its housing and struck the employee's right forearm, resulting in a fracture and laceration.

When the truck came to a stop, the employee lost his footing and fell out of the truck bed on the driver's side and struck the back of his head on the driveway pavement. The employee suffered a laceration to the back of the head and a skull fracture.
