A technician was troubleshooting a multi-spindle CNC lathe which is used for metalworking. The technician was manually spinning the spindle which causes the belt and pully to spin. The injured employee’s hand became caught in the machine resulting in an amputation to the left index finger.

Employee was driving a standard roll-off truck for picking up containers of spent material from various metalworking companies in the area. He was on-site to pick up a metal box type container from a customer. The employee hooked the truck's wire rope cable and winch up to the box and engaged the winch briefly.
