When the spindle came to rest on the floor, the load shifted and the tension bar struck one of the employees on the lower legs, knocking him to the concrete floor. He suffered a broken right hip and a broken left ankle and was hospitalized.

On 23 February 2015, at approximately 1030 hrs at the Flogistix manufacturing site in Pampa, Texas, employee was cutting a metal stud on a chop saw when the material kicked back and crushed the distal phalanx of the index finger (right hand). The employee was taken to Pampa Memorial Hospital where physicians opted to amputate due to the extent of the injury.

The employee was making adjustments to a fence staple manufacturing machine. The employee was working on the machine when the flywheel came down, catching his hand between the flywheel cutter and stationary cutter. The employee's left thumb was amputated and the employee was hospitalized.

While manufacturing a shaft pin on a metal lathe, an employee in the Bldg. #304 Machine Shop was sanding the rotating pin with an emery cloth. The cloth bound and pulled the employee into the rotating lathe machine, resulting in a left broken hand and lacerations on the right hand that required surgery and in-patient hospitalization.
