The employee suffered an L-1 lumbar fracture and was admitted to the hospital.

At this time, he used his upper body, shoulder area to loosen the section. The duct work then fell and pinned him underneath it. He suffered a lumbar fracture.

On October 4, 2017, at 3:00 p.m., an employee was using a tape measurer to measure a warehouse roof vent hole where the vent cover structure had been removed to place decking material. The tape measurer then fell onto the retrofitted drywall vent cover. The employee was trying to recover the tape measurer when the drywall collapsed and he fell 28 feet to the floor of the warehouse, fracturing his right hip, left foot, right cheek, and lumbar spine. He was hospitalized and had surgery.

On September 25, 2017, at 5:00 p.m. the injured employee was loading a semi-trailer at the dock of the facility using a forklift. The truck driver pulled away from the dock with the forklift still in the trailer section of the truck. The forklift and the employee fell out of the back of the truck onto the concrete parking lot. The employee suffered a lumbar fracture and a spinal injury.
