When the chain was removed, a section of pipe rack became unstable, causing the rack and the employee to fall approximately 3 feet from the flatbed trailer to the ground. The employee became pinned under the section of pipe rack and was hospitalized with a fractured pelvis. The section of pipe rack was 30 feet long and weighed approximately 1,380 pound

On August 17, 2021, an employee was inside a trailer that his truck had just backed up to a loading dock. As he tried to straighten a banded load of cardboard, the band snapped and the load fell, knocking him into the side of the trailer. He suffered head lacerations and a dislocated shoulder.
