An employee was retrieving a pallet from the upper shelving of the pallet rack (approximately 40 feet) using a pallet jack. As he raised the pallet, another pallet on the backside of the rack was overlapping and began to tip. As the second pallet tipped forward four wheel rims and the wooden pallet fell from the rack, one of the rims struck the employee's head. The employee sustained bleeding and swelling of the brain.

An employee responded to an alarm in the mechanical room. He noticed a loose belt flapping and turned off the motor to adjust the belt on the air handler. When he went to inspect the belt, his finger was caught between the spinning belt and pulley resulting in a fingertip amputation.

An employee was removing the nails securing two scaffolding planks together. A coworker had removed the support beam beneath the scaffold platform. When the employee removed the nail holding the two overlapping scaffold planks together, the planks collapsed, and the employee fell 8 feet to the concrete floor, face first. The employee suffered a broken right upper arm, broken cheek bones, and a broken nose.
