The pin caught the employee's left middle finger, partially amputating it.

An employee was cutting a piece of steel with a torch. About 40 feet away, an excavator with a grapple attachment was pulling apart two other pieces of steel that were connected with bolts. When those two pieces were pulled apart, the bolts broke and one of the pieces flew out the side of the grapple. It struck the employee in the hand and right leg, causing hand fractures and lacerations and a broken femur. The employee was hospitalized.

An employee was repairing a sand blaster machine. After the sand blaster was reassembled, the employee then turned on the generator to check the pressure. The employee went back to check the drum and air was escaping. The employee noticed that the hose was slipping off. The employee grabbed the hose and it slipped off and struck the employee on the left side of the head resulting in a laceration and a swollen left eye.

An employee was standing on the second step of a step-ladder unhooking a 4-inch hose. After unhooking the hose, the end of the hose caught his glove pulling the employee off the ladder to the deck two feet below the ladder. The hose continued to pull the employee to the ground approximately 2.5 feet below the stepladder. The employee suffered neck trauma and spinal cord swelling.

As an employee was disconnecting the hoses for the water, he was sprayed with residual pressurized water from a 6-inch coupling he was disconnecting at the truck. He suffered second-degree burns to both forearms and the left side of the face and neck.
