An employee was backing a trailer into a building dock when the right trailer door came unlatched and hit the building. The employee pulled forward and walked to the back of the trailer to re-secure door. The door then broke from the hinges and started to fall toward the employee. As he tried to move out of the way, he tripped and fell, landing on a chock block, fracturing three to four ribs and suffering a collapsed left lung.

On or about 12/16/15, an employee was opening the gate for a semi truck to leave the area. The semi trailer's rear bumper with the hazmat sign got hooked on the gate. The gate broke loose from the hinges and went flying 10 feet in the air landing on the employee. The employee was hospitalized with fractured ribs, a potential hole in the lung, and a facial laceration.

Two employees were working together, replacing hinges and remounting a dump trailer door. After welding the door, they checked the alignment and the weld. The dump trailer door fell off and hit one employee, injuring his shoulder, wrist, and pelvis.

The employee had removed/unlocked both sides, which caused the door to fall forward. The employee attempted to catch the door, and his right hand got caught between the metal mill door and a metal support beam. The employee was hospitalized and required surgery.
