Employees were attaching bungee cords to keep hides separated between carcasses. Two S-hooks on a bungee cord became entangled and began stretching. As an employee drew her knife to cut the bungee cords apart, the knife lacerated another employee’s left arm above the elbow.

An employee was placing bungee cords on beef hides before they reach the side puller. As the employee was placing the bungee cord on the hides, her glove got stuck on the bungee cord hook. As the line progressed, the employee was pulled around the guardrail and to the ground, resulting in fractures to the right tibia and fibula.

An employee was operating a fleshing machine when an animal hide became stuck in the roller system. As the employee attempted to remove the hide, the employee's left hand was pinched between a rotating chain and sprocket, partially amputating the third digit fingertip and avulsing the second and fourth digits. The machine was guarded and was not locked/tagged out at the time.

On January 26, 2017, at 3:00 PM, an employee was performing his rounds such as checking the hide conveyor for fats and paper towel build up to prevent a future problem. The conveyor was running and one of the hooks caught the employee's right arm tearing some of the muscle tissue from the arm. He was hospitalized where he had several surgeries due to E. Coli from the hide conveyor.

The injured employee was training another employee on how to operate the side puller machine. The injured employee noticed that a hide was not clamped properly and went to adjust it on the clamp. At the same time, other employees pressed the buttons to start the machine. The clamp caught the injured employee's right middle finger, which had to be surgically amputated from the tip to the first knuckle.

An employee was handling hides to get them in the deflesher when the "gripper" jaws opened and shut on the employee's left pinky. The employee sustained a soft tissue amputation of the left pinky finger.
