He sustained a crushing injury which required amputation of the right middle and ring fingertips.

An employee was shoveling gravel into a 1-foot trench by a partition wall. The site supervisor was operating a mini excavator near the partition wall while the employee was working on the opposite side. The mini excavator bumped into the wall, causing it to fall to pieces. Some of the pieces fell on and broke the employee's hip.

On October 23, 2015, at approximately 1:00pm, a an employee was hospitalized after suffering a compression fracture to his back and laceration to his head. The employee was driving an extendible boom manlift across a designated railroad crossing. The machine malfunctioned and stopped with the bucket portion of the lift above the railroad tracks. As a train arrived, the employee untied his harness and jumped from the bucket approximately 10' out and 6' down landing on a gravel surface.

An Ironworker was tying rebar 8 feet above ground. He hooked the rebar onto a wall with a hook and then leaned back and the wall hook slid off the rebar and the ironworker fell backwards approximately 8 feet to the crushed gravel ground injuring the right leg.

The company was having a United Way rally which was done once per year. They were performing a treasure hunt and the employee was going from one station to another. As the employee crossed the gravel parking lot, the employee slipped and fell breaking the right wrist.

Five employees were leaving the jobsite on an ATV designed for four passengers. The ATV was traveling down a steep hill on a gravel road when it fishtailed and began to roll due to the steepness of the grade. Two employees who were not wearing seatbelts were ejected. One employee suffered two broken ribs and was hospitalized. The other employees were treated and released.

The employer is contracted for all the ground and foundation work for a new grain facility. On the day of the incident the employer was having gravel delivered to construct a road. One of the side-dumper trucks got stuck in the ditch across from the site. In an effort to pull the truck free the employer used one of their Caterpillar D6 dozers and two 16 foot long 5/16 inch chains. After the first unsuccessful attempt to pull the tractor trailer free the dozer operator attempted another pull.

On July 17, 2015, at approximately 1:45pm, an employee was hospitalized after suffering two broken bones to his lower left leg. Before the incident, the employee was standing on the third rung of a 12-foot stepladder, tightening bolts on a flange with a torque wrench. While applying pressure to the wrench, a reducer on the wrench broke, causing the employee to lose his balance. As he stepped down 34" to the gravel surface, he landed awkwardly on his left foot, causing fractures.

As she curbed to the right, she did not realize that the hose unraveled and caught the tires, causing the wand to move and strike her right eye. This resulted in the loss of an eye.
