On October 23, 2015, at approximately 1:00pm, a an employee was hospitalized after suffering a compression fracture to his back and laceration to his head. The employee was driving an extendible boom manlift across a designated railroad crossing. The machine malfunctioned and stopped with the bucket portion of the lift above the railroad tracks. As a train arrived, the employee untied his harness and jumped from the bucket approximately 10' out and 6' down landing on a gravel surface.

One of the side-dumper trucks got stuck in the ditch across from the site. In an effort to pull the truck free the employer used one of their Caterpillar D6 dozers and two 16 foot long 5/16 inch chains. After the first unsuccessful attempt to pull the tractor trailer free the dozer operator attempted another pull.

On July 17, 2015, at approximately 1:45pm, an employee was hospitalized after suffering two broken bones to his lower left leg. Before the incident, the employee was standing on the third rung of a 12-foot stepladder, tightening bolts on a flange with a torque wrench. While applying pressure to the wrench, a reducer on the wrench broke, causing the employee to lose his balance. As he stepped down 34" to the gravel surface, he landed awkwardly on his left foot, causing fractures.
