On 9/21/2021, at 2:15 PM, an employee in a boom lift was setting a bar joist that was being swung in by a crane. While moving the bar joist across a girder, the joist rolled off the girder and struck the employee’s right arm. The employee sustained a fracture to the upper right arm requiring hospitalization and surgery.

An employee was assisting co-workers with re-lining a steel ladle cover for a ladle that was approximately 12 feet by 14 feet. The ladle was attached to an overhead double girder bridge crane and was suspended off the floor. The ladle lid moved off the crane and landed on the employee's foot, resulting in broken bones.

Employees were using an overhead crane to submerge metal I-beams and girders in a galvanizing dip tank. Other I-beams and girders were stacked on the shop floor waiting to be processed. An employee was delivering roof trusses to a new residential home site. The delivery truck bed was on an angle allowing the trusses to slide off the truck bed.
