On February 11, 2019, at 1:30 p.m., a journeyman mechanic was performing a diagnostic test on a tractor (truck). While standing inside the engine bay of the truck between the frame and the left steer tire axel, the vehicle started and the truck moved forward pinning the employee under the steer tire. The employee's upper leg was crushed and the hip and ankle were fractured.

The employees were walking and working from a partial bundle of roof sheeting (approximately 3 sheets). During the operation, an employee stepped on an inadequately supported area of the roof sheeting partial bundle; the sheets bent down and the employee fell 21 feet to the ground below. The employee was hospitalized with a broken arm and tailbone.

On August 8, 2018, employees were rigging an engine coupling on a ship when the tension in the chain fall released and the load shifted, pinning the injured employee's arm against the ship's piping. The weight of the load (coupling) was 4,800 lbs. The employee's right arm was crushed, fracturing the radius and ulna and requiring surgery.

An employee was loading the engine pistons and cylinder liners onto a pallet to remove from the area for housekeeping purposes. He lifted the last piston by placing his fingers into the large holes on each side of the piston. The piston did not have rings installed on it. When he sat the piston (98 pounds) on the liner it slid into the liner catching two of his fingers between the piston and the liner, amputating the tips of two fingers on his left hand.

The employee was performing repairs on a fire truck while the truck's cab was being held up in a tilted position by a safety latch. The pivot point on the cab broke and released the cab lock. Two employees were trying to loosen a bolt to disassemble a coupler between an engine and compressor. One employee was on the engine side while the other (the injured employee) was on the compressor side holding a shim. When the employee on the engine side turned the bolt for the coupling assembly, the engine crank rolled and amputated the injured employee's left middle fingertip.

An employee was standing on the engine compartment of a manlift lining up the holes on the boom to apply the pin. As another worker hit the pin, it vibrated and caused the employee to lose his balance. He fell 7' 3" to the ground, fracturing his right wrist, dislocating his left collarbone, and suffering two contusions to the back and front of his head. He was hospitalized.
