At 9:30 a.m. on August 4, 2021, a two-person crew was trimming branches on a 36-foot locust tree using an elevator lift truck. An employee was in the elevated bucket of the boom truck using a stick saw. While he was lowering the boom to clear some hangers, the walking pin to the hydraulic cylinder failed. The upper boom and bucket fell about 7.9 feet, coming to rest on the top of the cab protection rack. The employee suffered a head laceration and was hospitalized.

While the 29th joint was being laid down, the employee's right ring finger was crushed between the tubing and the floor as the joint bounced resulting in amputation.

The door then bounced back up and came down onto the employee's leg. The employee sustained a fractured tibia and fibula, compression fractures to vertebrae, and a collapsed lung.

On June 6, 2019, a grain elevator operator was cleaning out a grain bin using a long scraper to scrape the residual grain from the edges while standing on a ladder. The employee began descending a step ladder into the elevator shaft pit when the employee fell from the ladder onto a steel pipe at the bottom of the pit, fracturing the right femur. While lifting a single joint of drill pipe, it fell out of the elevator and came down on the injured employee's left hand which was on the handrail.
