On November 5, 2021, at approximately 8:30 PM, an employee was operating a reach truck to dispose a scrap walkie-rider into an open-top dumpster. The reach truck was operated on an uneven and damaged surface, with an elevated load, causing it to overturn. The employee's head impacted the concrete surface, resulting in a head contusion and five broken ribs.

At 9:30 a.m. on August 4, 2021, a two-person crew was trimming branches on a 36-foot locust tree using an elevator lift truck. An employee was in the elevated bucket of the boom truck using a stick saw. While he was lowering the boom to clear some hangers, the walking pin to the hydraulic cylinder failed. The upper boom and bucket fell about 7.9 feet, coming to rest on the top of the cab protection rack. The employee suffered a head laceration and was hospitalized.

The forklift backed into a safety bollard. The cage broke free as the chains and padlock snapped. It fell to the ground and the employee suffered a broken back with spinal cord damage.

The employee was descending a fiberglass step ladder when the ladder shifted from beneath him in one quick movement, causing him to lose his balance and fall approximately 5 feet. The employee suffered an L-1 lumbar fracture and was admitted to the hospital.

The employee fell to the concrete floor about 10 feet below, suffering a back/spine injury. The employee was hospitalized.

The employee suffered a dislocated left elbow, broken left femur, single vertebra fracture, and bleeding in the brain, requiring hospitalization and surgery.

It is believed that the injured employee was inside the catering box while it was raised to service the aircraft and fell out after the box's door was opened.
