An employee was working in a test booth that tests the noise quality of automobile seats. The conveyor line stopped moving. The employee looked through the test booth doors, leaned over and placed their left hand on the chain and sprocket that drives the conveyor. The conveyor system began moving, resulting in amputations to the left index, middle and ring fingers.

An employee was helping to change out a cylinder on a press. He was putting the cylinder arms down to put the cylinder in place. As he stepped over the driveshaft to pull the pin and roll the cylinders, he lost balance and started to fall forward. He dropped the cylinder arm to regain his balance and the arm came down on his right index finger and amputated the fingertip.

Employees were driving piles. An employee was using his left hand to guide the leads (upright portion that drives the hammer). The lines were suspended by a crane and the third line was not in use. As the crane lowered the leads, the weight of the leads draped onto the third line. This caused the third line to tighten against the employee’s hand causing an amputation to the left thumb just below the nailbed near the first joint.

An employee was underneath a disabled tri-axle dump truck attempting to release a driveshaft from the bound-up transmission. When the driveshaft was disconnected and came off, the truck drifted and dragged the employee who was making the repairs. The employee was hospitalized and suffered a broken collarbone, four broken ribs, and a bruised kidney.

An employee was operating the waste chopper. The auger of the waste chopper is driven by two belts that are on a single wheel/sprocket; the belts and the motor that drives them are locked in a cabinet. As the employee was accessing the cabinet to retrieve a tool, his right thumb was caught in a pinch point between the belt and pulley, resulting in amputation. The machine was running at the time.

In the planner area, an employee was piling lumber that was being transported by a lumber chain. One of the chains became jammed. The employee reached between the chain way because the chains were doubled over. Energy was stored in the gear box due to the jam. The employee manually tugged the belt associated with the belt/pulley mechanism for the 2 hp motor and gear box. The motor/gear box drives the chains. When the employee tugged on the belt, it released the stored energy from the jammed

An employee was performing troubleshooting activities, trying to free a traverse chain. The employee attempted to turn the traverse motor, which drives the chain/conveyor, on and off. The traverse motor jumped 2 feet and impacted the employee's hand.
